Bringing hospital-level care into the home

Hospital at Home delivers hospital-level care to patients with acute needs in their own homes, improving patient experience while lowering the cost of care. But scaling it depends on technology — remote monitoring, telemedicine, care-team coordination, and supply-chain fulfillment the electronic health record was never built to handle.

WHAT HTL DID

A technology advisor to Hospital at Home programs

As technology advisor to the Home Hospital programs, HTL collaborates with clinical teams to discover, evaluate, and pilot the technology that makes home-based acute care possible — remote patient monitoring, telemedicine, distributed clinical-team coordination, and supply-chain coordination — using human-centered design, technology advising, and operational piloting.

AREAS OF WORK

Four fronts of home-based care

Remote patient monitoring
Market analyses and rigorous testing of dozens of devices — patches, arm bands, wearables — to determine which reliably monitor vital signs, falls, and arrhythmias at home, and to set the quality and safety standards for choosing among them.

Telemedicine
Remote-communication tools that connect patients with their hospital care team, giving patients confidence that a rapid response is always on call.

Distributed clinical-team coordination
Shared task management and seamless messaging for clinicians in the field, plus route planning and coordination with Mobile Integrated Healthcare paramedics — needs the EHR can't meet.

Distributed supply-chain coordination
Technology-enabling order-to-fulfillment across pharmacy, infusion, respiratory care, diagnostics, durable medical equipment, and transportation as programs scale in volume and geography.

An electronic safety net for care at home

By validating the monitoring, communication, and coordination technology home-based acute care requires, HTL is building an electronic safety net that lets Home Hospital programs expand, bringing more patients the option to recover safely at home.
