Transforming Care Delivery with Hospital at Home.
Dr. Leff is Professor of Medicine at the Johns Hopkins University School of Medicine, where he is the Director of the Center for Transformative Geriatric Research. He holds joint appointments in the Department of Health Policy and Management at the Johns Hopkins Bloomberg School of Public Health and the Department of Community and Public Health at the Johns Hopkins School of Nursing.
As an internationally recognized leader and researcher Dr. Leff’s research focuses on novel models of care delivery for older adults, issues related to multi-morbidity, risk prediction, performance measurement, and quality measurement and improvement, with an emphasis on home and community-based models of care including Hospital at Home, home-based primary care, CAPABLE, and others.
